1. Oxygen use. Mrs. F needs to be put on the oxygen she was ordered to have at the time of discharge. Perhaps home health can be contacted and set this up, with regular checks to make sure she has an adequate supply for outings and her everyday needs. Additionally, Mrs. F should be educated about the reasons for her being put on oxygen, and the benefits of use, etc.
2. Poly-pharmacy. Home health will be able to assist Mrs. Fisher with her new and continuing medications since her hospital discharge. Poly-pharmacy is a common issue with the elderly and usually in-home assistance help the patient to take their prescribed medications, when due.
