Community Assessment X Cleveland State University Community Assessment X Berea is a city of Cleveland, a major city in the state of Ohio, and a western suburb in Cuyahoga County. Based on community assessment data, a critical community nursing diagnosis for increased infant mortality rate related to prematurity, low birth weight and birth defects was identified and goals were developed while considering primary, secondary and tertiary levels of prevention. A community health nurse would collaborate with health care professionals that focus on woman and infant health.
