Different standards of medical records documentation have different compliance plans that correspond to them. Staff members must follow all billing rules and medical facilities have to be certain that this is taking place. When filing medical records they have to be accurate: they have to contain the patient's conditions and diagnoses, and they must trace the patient's course of care, also they have to have the patient medical history, including family history. Medical records of patients are legal documents; physicians may use them in their defense if they are accused of improperly treating patients.
