Blood Transfusion

In class we have learned that informed consent the idea or notion that healthcare decisions should be made in a collaborative matter and between the patient and healthcare team.(Bustillos, Sec 4.2) The five requirements for a consent form are adequate disclosure of information, decisional capacity of the patient, the patient's comprehension of information, voluntariness and consent of the patient. In this paper I will be discussing how each one fits into the consent form for blood transfusion.
