In a recent Indiana State Board of Health survey, SouthernCare Hospice Kokomo Branch, was cited for failing to properly document wounds on clients who reside in nursing homes. The purpose of this consultation is assess the current nursing policies, processes, and procedures, and recommend appropriate interventions that will improve patient outcomes by ensuring proper treatment and documentation of patients with wounds.
