Mr. B should have also had a healthcare professional monitoring him continuously at the bedside while he was sedated. According the hospitals conscious sedation policy, Mr. B should have been on cardiac monitoring along with continuous blood pressure monitoring and pulse oximetry.
- (Why The Event Happened) Mr. B was not properly monitored after being sedated. Once his oxygen saturation levels started to decline, the issue was not addressed immediately which led to a further decline.
